After Hours and Weekends:
There is always one of our care team members on call to assist you if you have concerns about your child. After hour call operates from 4:45 p.m. - 8:15 a.m. Monday thru Friday, and all weekend starting at 4:45 p.m. on Friday.
For after hours and weekend needs, please call our office number and follow the prompts to speak to the on call care team member. Please note; a $30 charge will be applied to your account when this service is utilized.
Regular well-child exams are an invaluable time for parents and providers to focus on preventive care to help keep your children healthy. Each visit includes a complete physical examination to assess your child’s growth and development, as well as to help identify problems early. Hearing and vision screening, vaccinations, and other tests will be a part of some visits.
Well-child visits are key times for communication. Expect to be given information about normal development, nutrition, sleep, safety, and other important topics for parents. It is an ideal time for you to ask questions about your child. Make the most of these visits by writing down your most important questions and concerns to bring with you.
The scheduled intervals for well-child exams:
- Newborn Visit (2-3 days after discharge from the hospital)
- 2 weeks
- 1 month
- 2 months
- 4 months
- 6 months
- 9 months
- 12 months (1 year)
- 15 months
- 18 months
- 24 months (2 year)
- 30 months
- 3 years - once every year thereafter for an annual comprehensive physical exam
In an effort to help you become familiar with Dr. Middleton, Dr. Acosta, Christine May, ARNP, Briana McLain, ARNP, Lucy Aitcheson ARNP and Lauren Freeman, ARNP, well-child visits through 18 months will be scheduled alternately with a physician and a nurse practitioner. After your child turns 18 months, you may schedule well-child visits with the provider of your choice.
Between scheduled well visits, appointments should be made any time your child seems ill or whenever you have concerns about your child’s health or development. We offer a walk-in time daily, from 8:15 a.m. to 9:00 a.m., in order to quickly assess your child for acute illnesses that have emerged overnight. More complicated illnesses or ongoing symptoms should not be seen during walk-ins time slots in order to allow adequate time for discussion. After this walk-in time, all visits are by appointment.
Insurance and Billing
Middleton Pediatrics accepts the following insurance plans. The parent or legal guardian accompanying a child is responsible for any copays, deductibles, coinsurance, or fees not covered by insurance at the time of the appointment. It is the responsibility of the parent or legal guardian to verify that Middleton Pediatrics accepts your insurance before making an appointment. Please contact your insurance company directly to determine if Middleton Pediatrics is in-network with your family’s plan.
*BCBS of FL: We will not be in-network with the Florida Blue: "myBlue HMO" plan as of 1/1/2018. If you are selecting new insurance for 2018, we do not participate in "myBlue HMO".

Payment Options for Share Plans and Self-Pay Accounts
Self-Pay: We accept checks, cash or credit cards. Payment is due when services are rendered and putting a credit card on file is required. You will be supplied with a price guide and vaccine schedule. Please ask about options for a self-pay discount, vaccines and payment plans.
Share Plans (Christian Care, Medi-Share, Liberty Health Share, and any others): It is required that a credit card be kept on file. You will be supplied with a price guide and vaccine schedule. We are happy to file the plan for reimbursement, however, payment for uncovered services will need to be settled within 30 days. We only offer a discount if no discount was given by the share plan. Please ask about options for vaccines and payment plans.
